All information will be used only to assist in your training and will be held as confidential.

Please describe any medical condition that may affect your or your child's training or that should be known in an emergency - including joint or muscle problems, neurological conditions, drug allergies, asthma, or medication you may be taking, if applicable. This information will be kept confidential and will not affect your or your child's ability to train with us!

On occasion, for special events and promotions, Catonsville Seido Karate may take pictures of its students and use students' first names, age groups, and ranks on its websites and/or social media related to Catonsville Seido Karate and the Worldwide Seido Karate Organization. Please indicate your image sharing preference for the student on this waiver:

Participation in our classes and events is subject to the following terms and conditions:

• No refunds.

• Participant agrees to follow all safety rules and instructions.

• We reserve the right to refuse participation to anyone for any reason permitted by applicable law.

• Martial arts and self-defense training are contact athletic activities with inherent risk, including the risk of serious injury or death. As partial consideration for being allowed to participate and receive instruction, you agree to release all claims of liability and to indemnify and hold harmless for all claims resulting from your participation: 1) all persons giving instruction in this activity; 2) all training partners and fellow participants; 3) all representatives and members of the World Seido Karate Organization; and 4) any person, group, legal entity, or organization providing related facilities or services.

• Participation is at your own risk. By participating or allowing your minor child to participate, you indicate your agreement to these conditions.
